Required Documentation to Utilize Disability Services

Disability Services provides services for full- and part-time undergraduate and graduate students with permanent or temporary disabilities. Students with medical conditions are also encouraged to contact the Center for Health and Wellness, where nurse practitioners and part-time physicians can help monitor their particular situations.

To register with Disability Services, documentation must be current and must be submitted by a licensed or certified diagnostician or medical professional. This documentation must be a comprehensive assessment including recommendations and rationale for accommodations as well as recommendations for treatment. Guidelines for providing documentation of a Learning Disability are below, followed by documentation guidelines for students with Attention Deficit/Hyperactivity Disorder, Psychiatric Disabilities, Deaf and Hearing Impairments, Blind and Low Vision Impairments, Mobility/Orthopedic Disability and Systemic Disorders, and Medical Conditions:
