Please submit your most recent assessment/evaluation/letter from your treating specialist, as well as a copy of your most recent education plan (IEP, 504 Plan, letter from private school) and SAT/ACT approval letter (if applicable) by emailing Stephanie Segalini, Steph Bohler and Katie Johnson.
In addition, we recommend that if you have a medical condition, you should contact the Health Center where nurse practitioners and part-time physicians can help you maintain your health.
The purpose of medical documentation is to establish:
- The presence of a disability
- The symptoms of the disability
- The impact of the disability
SAS uses this information to:
- Determine what barriers you may experience in academics
- What reasonable accommodations are appropriate and necessary for access
Medical documentation needs to be:
- Current (within the last 6 months)
- Typed on letterhead
- Hand signed
- From your licensed, treating doctor
Or
- A recent neuropsychological evaluation for learning disabilities (within 3-4 years old)
Documentation must include:
- Diagnosis
- History
- Symptoms
- Impact to functioning/barriers experienced
- Prognosis/treatment
