Utilizing ServiceNow, IT, Facilities and Human Resources have worked together to create new forms and enhance the onboarding and exit process.

New Hire Setup

When preparing for a new employee, access the New Hire Setup form by going to the IT Service Portal and searching for "New Hire."

Move Request

When an employee is changing locations or departments, access the Move Request form by going to the IT Service Portal and searching for "Move."

Vacancy

When an employee is leaving the University, access the Vacancy form by going to the IT Service Portal and searching for "Vacancy." This includes services such as the move or return of furniture and computer equipment and the termination of access to applications and systems.

Name of Form Purpose When to submit?
New Hire Setup form Use this form when a new Faculty or Staff member has been hired to request a computer, telephone, voicemail, access to a printer, software, application, account, distribution list addition, shared calendar access, painting/patching and furniture requests, assignment of mail stop, physical keys needed, building or room swipe access needed, etc. 2 weeks in advance. This is of even greater importance when computer equipment needs to be mailed home prior to their official start date.  Please note swipe access cannot be granted until 1 business day after the new employee’s official start date.
Move request form

Use this form when a Faculty or Staff member is relocating from one building or office to another location on campus – even within the same office suite. This must be done even if the employee moves their equipment on their own due to the phone needing to be relocated and the facilities database being updated.

2 weeks in advance. For larger moves involving multiple members, please also contact Facilities at least 30 days in advance of the scheduled move date in order to coordinate the move. In addition, requests at peak times such as the beginning of each semester may require rescheduling due to resource limitations.

Vacancy form Use this form when a Faculty or Staff member is exiting Bentley University for any reason (whether voluntary or involuntary). 2 weeks in advance.
